Job Summary
As a Case Manager, you will serve as a vital link between clients, regional centers, service providers, and our internal departments. Your role is to ensure that each client’s services are aligned with their individual program plan, their budget is being used effectively, and that questions are answered with clarity and care. This is a dynamic position requiring empathy, attention to detail, and strong communication skills.
Key Responsibilities

Serve as the primary point of contact for an assigned caseload of clients and their families.

Guide clients through onboarding, service planning, and ongoing program support.

Review and monitor client budgets, ensuring service authorizations and expenditures stay aligned with their approved spending plan.

Collaborate with regional centers, vendors, and internal departments (e.g., payroll and compliance).

Maintain accurate and timely records of all interactions, documentation, and case notes.

Respond to client inquiries via phone and email, typically within 24 hours.

Proactively identify potential risks (e.g., overspending, service gaps) and work with clients to resolve issues.

Educate clients and caregivers on FMS policies, self-determination regulations, and their responsibilities under the program.

Participate in team meetings, trainings, and ongoing process improvement efforts.
Required Qualifications

Bachelor’s degree in Social Work, Human Services, Psychology, Business, or related field (or equivalent work experience).

At least 1 year of case management, care coordination, or client services experience.

Excellent written and verbal communication skills.

Strong organizational skills and ability to manage multiple priorities.

Familiarity with California’s Self-Determination Program or regional center services (preferred but not required).

Tech-savvy with proficiency in Microsoft Office or Google Workspace and the ability to learn internal platforms.
