1st Approach We Follow
Initial Contact and Referral
Submit your completed Referral form and Spending Plan via email or by contacting us through our contact form. Our team will review your information and contact you or your representative to review.
2nd Approach We Follow
Review & Approval
We’ll work closely with your service coordinator, independent facilitator, and regional center to confirm your eligibility, start date, obtain a finalize your spending plan, and address any questions or concerns.
3rd Approach We Follow
Support & Delivery
Once all documents are received and approved, you’ll receive confirmation and access to your client portal and resources. Our team will ensure all venders and employees are onboarded prior to your start date in order to begin working with your chosen providers and enjoy the benefits of the Self Determination Program!
