1st Approach We Follow
Initial Contact and Referral
We will gather the essential details about your organization or services. These include:
- Applicable License or Certification
- W-9 Tax Form (to ensure accurate tax reporting)
- Proof of Business License
- Service Agreement (provided by Sentinel Four)
All forms and documents will be sent via Docusign for secure and convenient completion. Please submit these promptly to avoid delays.
2nd Approach We Follow
Review & Approval
Once documents are submitted, our team will review them to verify compliance with program and regulatory requirements. This step includes:
- Verifying licenses and certifications, as applicable.
- Ensuring the W-9 information is accurate and complete.
- Approving service agreements in alignment with the participant’s budget and plan.
We may reach out to clarify any details during this step.
3rd Approach We Follow
Support & Delivery
Once onboarding is complete, you’ll be ready to provide services to participants under the Self-Determination Program. Sentinel Four is here to support you with:
